This medallion is encountered in the TDP/TG mission "The Lost City". There are two of them, each located on the remains of Keepers sent to hide the Talisman of Fire, and also apparently to map the ruins of Karath-din.
Garrett needs to find these medallions on hard and expert settings, as the Keepers would appreciate their return.[1]
